What is it?

In Bexley, we are currently delivering a fun and educational virtual online service, by nutritionists, to help you and your family make informed choices on lifestyles changes to ensure your child or children are a healthy weight. The service is made up of 8 weekly, 75 minute, 121 or group sessions( online or by phone), and covers topics such as:

  • Understanding food labels
  • Sugary drinks
  • Portion sizes
  • Cooking on a budget
  • Reducing sedentary behaviour; during lockdown and beyond
  • Weekly recipes to cook at home
  • Live cookery class to follow at home

These sessions are for families with children, above the healthy weight range, living within Bexley borough and aged 4-11 years old. The programme is designed to inspire and help you feed your child healthy and nutritious meals; we also provide you with delicious recipes to try at home.

The programme also includes sessions designed specifically to encourage the children to try more cooking. These sessions are run throughout the 8 week course, allowing children to take part in an hour’s cookery completing recipes and enjoying cooking, helping them get more kitchen skills and make new friends, whilst the parent / Guardian learns new nutrition facts and tips.

Please note this service may change to a face to face group sessions once COVID-19  restrictions in the United Kingdom change, in accordance to Government regulations.

Everyone Health delivers a free healthy eating and lifestyle programme to children and young people aged 5 – 19 called Alive ‘N’ kicking. This programme is also available to those up to the age of 25 who have special educational needs or disability (SEND).

About the Alive ‘N’ Kicking Programme

We understand that it is challenging for families to stay on track with their healthy eating and fitness goals. Our programmes are run by nutritionist and physical activity specialist who will work with you and support you to make long-term healthy changes as a family. Eating and drinking well can be a fun experience and it can also help children and young people developing a love of good and healthy food as well as develop some social skills.

Due to coronavirus restriction our team have adapted the ANK service to support people virtually via video calls and telephone.

What we offer?

Tailored one-to-one support and weekly interactive webinars and posts on healthy eating and physical activity via Facebook and Instagram.

Inclusive non-judgemental environment and a family-friendly and flexible timetable that is tailored to families’ individual needs.

Topics covered over the 6 to 8 weeks:

  • Eatwell guide and balancing what we eat – to help families to get a balance of healthier and more sustainable food.
  • Portion sizes and regular meal planning – guidance on what, how much and how often we should eat
  • Mindful eating and managing triggers – to understand what cause certain behaviours and be able to identify individual triggers
  • Keeping active and having good sleep – explore ideas to increase activity levels and develop bedtime routine to promote quality sleep 
  • Understanding food labels and sugary drinks – so that families can identify ingredients on food labels such as sugar, fat, salt, and can make healthier choices
  • Healthy cooking methods, cooking on a budget - enjoyable activities that can be done at home such as weekly recipe challenges to develop practical skills and maintain a healthy lifestyle.

Alive ‘N’ Kicking was fun! I have learned a lot from this programme. I used to make poor breakfast choices and our portion sizes were far too big. We really enjoyed the portion sizes session. I realised we were eating way too much of everything and needed to cut down. We also learned some strategies, such as setting up the food environment to encourage healthy eating by limiting chocolate, cakes, sweets and having more fruits and vegetables at home.

What is Sandwell’s Stop Smoking Service?

Sandwell’s Stop Smoking Service offers a 12-week service, to support you to quit smoking.  As part of the service, you will have a choice of a variety of nicotine replacement treatments (NRT), such as patches, gum, inhalators etc.  It is proven that by quitting smoking you improve your health and life expectancy.  During lockdown restrictions we continue to support all our client via telephone.

Who can Join?

Simply anyone who lives, or works, or has a GP in Sandwell. 

Why should I contact Everyone Health?

It’s not only an opportunity to put right potential health problems, but to get personalised one to one support to quit and keep smoke free in the future.

You will be assigned your very own expert stop smoking practitioner for 12 weeks.  You will be given free nicotine replacement treatments as well as support and advice. 

What happens when I start?

You will receive weekly or fortnightly appointments which will take about 15 - 20 minutes and will include:

Everyone Health is commissioned by Warwickshire County council to help support people across Warwickshire to improve their health and wellbeing, this service is called Fitter Futures.

Fitter Futures provides a weight management and Physical activity service on referral.

For members of the public to be eligible for this service they need to be registered with a Warwickshire GP and will need to meet a specific criteria dependant on which service it is.

Physical activity on referral

This is a 12-week programme which is delivered by a qualified fitness instructor who will devise you a Physical activity/healthy lifestyle programme.

Opening times: Open various times depending on location and provider

Eligibility:  You must be registered with a Warwickshire GP and have one of the following conditions:

  • mental health condition - such as mild to moderate depression, anxiety, low mood and mild cognitive impairment
  • respiratory condition
  • musculoskeletal condition
  • strength or balance concerns
  • neurological condition

  • diabetes (type one or two)
  • pre-diabetes
  • overweight/obesity (including in all stages of pregnancy and postnatal)
  • osteoarthritis, rheumatoid arthritis, or osteoporosis
  • dementia early/mid stages
  • recovering from cancer

Cost: Discounted costs which vary dependent on provider

Adult weight management

Our Everyone Health programme supports behaviour change around healthy eating, weight management and physical activity to manage weight loss and maintain this long term. 

Everyone Health fitness groups run once a week for 12 weeks and each session lasts for 90 minutes. You can join at any time as groups run on a rolling basis. Each group consists of about 10-15 people who are also looking to lose weight and feel healthier.

Each group session has two parts; the first part focuses on healthy eating, topics include portion sizes, eating out and food labelling. The second part is an activity-based session, this will allow you to improve your health and fitness in a safe and encouraging environment.

The sessions are delivered by a team of nutritionists and physical activity specialists who will help you lose weight permanently by making small but significant changes to their lifestyle, avoiding unrealistic and gimmicky diets.

The programme has been designed with pregnancy and the postnatal period in mind and, under these circumstances, additional support is available.

Eligibility:  

  • Must be registered with a Warwickshire GP
  • Aged 12 or over with body mass index (BMI) of 30 or more or a BMI of 28+ if there are complex needs.
  • Can only participate in one 12-week programme within a 12-month period.
  • Cannot have been a paying member of the service provider within the last 3 months.
  • Maximum of two referrals per person

We, Everyone Health Ltd, are a company incorporated in England and Wales. Under Data Protection legislation we have a legal duty to protect any information that we use or collect from you. We take measures to safeguard your data and apply security standards and controls to prevent any unauthorised access to it, we are committed to ensuring that your privacy is protected. Here, at Everyone Health Ltd, we will act as ‘controllers’ of the information we collect about you (‘personal data’), unless, we are contracted to act as data processors. Where we are data processors it will be clearly communicated to you who is the data controller.

The collection and use of this type of data is subject to strict controls.  Similarly, information about criminal convictions and offences are also limited in the way it can be processed. 

We will collect your personal data mostly through our contact with you and the data is usually provided by you but in some instances, we may receive data about you from other people, health services or organisations.  We will explain when this might happen in this policy.  

3. What Information Do We Collect & Why?

We are committed to protecting your personal data and will only process the data if we need to for a specific purpose and providing we have a legal basis, as explained below.

There are various legal bases on which we may collect and process your data.  We may have your consent, for example, you have informed us that you are happy for us to process your information for a specific purpose such as providing a service to you or receiving further information about what we do.  Please note we may not be able to enter into a contract with you in the absence of data.

Sometimes there is a contractual reason such as when we are commissioned to do the National Child Measurement Programme (NCMP), here the data results are closely protected with no information about individual children’s weights or heights disclosed to school staff or other pupils. Everyone Health store this data securely. Parents will be informed of their child’s results and aggregate data will be shared with the Department of Health. We recognise we are the processor of NCMP data and that the controller is NHS Digital.

There are also legal reasons for sharing and processing data, this is covered by GDPR Article 9.2 (h) – The processing is necessary for the purposes of preventive or occupational medicine, for the assessment of the working capacity of an employee, medical diagnosis, the provision of health or social care or treatment, or the management of health or social care systems and services.

Other legal reasons for collecting or sharing data, such as for employees when we have to collect the information for the HMRC, or, should you have an accident, we may need to provide details of this to the relevant health and safety authorities. Sensitive health data.   We collect any personal health data you provide to us when registering and signing up for our health services.   We collect this information to ensure we are offering you the right services and so your progress can be tracked by yourself and us. We may ask you for information about your health in order to recommend appropriate exercise regimes or offer our other services

Other Sensitive dataWe are sometimes required to collect information about your ethnicity and other sensitive data in order to provide aggregated reports to your local authority or commissioning group.  This information is used only for statistical purposes and is always kept secure. If you prefer not to provide us with this data we will not hold this data.  
Information about website visits including IP addressWe use your IP address to capture information about website visits; so we can learn more about the use of the website, in order to find ways to improve the website and our services for your benefit.  
Your communication preferencesWe keep a record of any permissions and preferences you give us about what types of communication you are happy to receive from us.  

Specifically, for our Staffordshire over 50’s contract, personal Data will be shared with the Council, Midlands and Lancashire Commissioning Support Unit (MLCSU) and Centre for Health and Development (CHAD)  for evaluation and longer term follow up purposes. Personal Data will be in an electronic format and shared securely as and when required by the Council and upon Termination of this Agreement/Contract.

4. Cookies

When you visit one of our websites, we collect standard internet log information for statistical purposes.

  • We use cookies to collect information in an anonymous way, including the number of visitors to the site, where visitors have come to the site from and the pages they visited.

5. Data Relating To Children

Our services are used by people of all ages. Everyone Health may accept website registrations and collect personal information from individuals under the age of 16. If you are under 16 we do not allow you to post information about yourself in any Everyone Health forums or community areas. Everyone Health accepts no liability if this instruction is ignored.

Children aged under the age of 16 can consent to their own treatment if they’re believed to have enough intelligence, competence and understanding to fully appreciate what’s involved in their treatment. This is known as being Gillick competent.

Otherwise, someone with parental responsibility can consent for them.

This could be:

  • the child’s mother or father.
  • the child’s legally appointed guardian
  • a person with a residence order concerning the child
  • a local authority designated to care for the child
  • a local authority or person with an emergency protection order for the child

Proof of age maybe required and retained for access to some of our health services.

6. How Do We Store And Protect Your Personal Information?

These are the basic guidelines we use to look after your personal data.

  • We maintain secure systems to protect your personal information
  • We respect your wishes about how we contact you, whether by post, telephone, email or text message
  • We will update your information or preferences promptly when you ask us to
  • We will respond fully to requests from you to see the information that we hold on you.
  • We will not hold your personal information for longer than is necessary. The amount of time data is kept before being disposed of will vary depending on why it was collected, how it is used, and in line with any applicable UK laws. We follow strict procedures when storing or handling information that you have given us.
  • All staff who are using information also undertake regular training to comply with policies and procedures around data protection, information security, confidentiality and the safe handling of information.
  • We will never sell your personal information to a third party.

7. Retention Policy

We retain personal information as long as is needed to comply with our legal obligations.  Where data is not needed for legal or statutory purposes we will delete this information.

The retention of information is in line with The Records Management Code of Practice for Health and Social Care 2016, namely this is 8 years for adults and 25 to 26 years for maternity, midwifery and children services. However in the event of any incident the retention period may be extended for legal purposes.

8. Services Provided By Contracted Third Parties

Everyone Health may share information with third party organisations that provide specific services on our behalf. These organisations act as a Data Processor under our instructions.  They may process data securely outside of the EU. There is a data sharing agreement ((DSA) contract in place with each third party which includes strict terms and conditions to protect your privacy.

Our current processing partners include Systm One, Bionicle, Gladstone, Refer-all, Oviva, Health Diagnostics and Everyone Active all of whom have their own privacy policy.  

9. Health Partners

Everyone Health runs services on behalf of Local Authorities, NHS, Clinical Commissioning Groups and Trusts.  These services are often run under a contract agreement.  Data may be shared with these organisations at a summary level but not at a personable identifiable level unless we are not the Data Controller or we are commissioned otherwise. With some of our health and well being services, with your consent, we may share identifiable information with your GP, NHS services and Commissioners of the service.

At the end of the management contract, if the service is to be run by another provider, Everyone Health will forward on your details to the new provider so they can continue to provide the service to you without interruption. If you do not wish us to do this please send a request to Everyone Health Head Office. These organisations will be a Data Controller in their own rights, and where they are processers of your data they will inform you directly or through their services such as a website about the data they hold and what processing they undertake.

10. Marketing

Everyone Health will never sell your personal information to any third party for marketing or other purposes.

In some cases, such as our Exercise and Referral  service, Everyone Health works in partnership with other organisations, such as Everyone Active, to provide services to you.  In these cases, the partner may contact you for legitimate purposes of providing health and well being services. These services will have their own privacy policy.

11. How Do We Use Your Information?

We use your information to help us provide and improve our services for you. We may use your information in the following ways.

  • to provide you with any services that you have with us as part of your health or well being
  • to report to our commissioners on how the services are performing – note: this will be using anonymised, pseudonymised or aggregated data, never using identifiable personal data unless we are contracted to do which will be made clear to you at the point of initial assessment.
  • to check your identity
  • to check your eligibility where appropriate
  • to update our records with any new information you give us
  • to notify you if we will be unable to provide a service you have booked before
  • to provide marketing communications (if you have given us your permission)
  • for research and analysis so we can develop and improve our services for your benefit
  • to tailor our communications to you to ensure relevance (if you do not want us to do this please contact us using the details below)
  • to comply with legal requirements.

12. Keeping You Updated

There are certain communications we need to send to you so we can provide our services.  We call these service communications and include for example notices about your registration, confirmations, appointment reminders, motivation announcements and waiting list announcements. 

We may from time to time contact you about our services or products we think you might find interesting by email, by post, telephone or SMS, but only if you have given us your permission to do so.

If you do not want us to contact you other than for service emails let us know when you next visit us or contact us using the details below. You may also opt-out of email or any other communications by contacting the support team at clinical.contactcentre@nhs.net or datacontroller@slm-ltd.co.uk, or by letting us know at one of our health service groups.

You have the right to know what personal information we hold about you. This is called a Subject Access Request.

Removing your data

If you no longer use our services and products and wish us to delete your personal data we will do this if there are no legal or statutory regulations requiring us to keep this information.

What is it?

While its great news that smoking rates among adults are consistently on the decline, there is still significant work to do in order to try and remove this incredibly dangerous habit out of society completely.

According to the most recent study, 15.8% of UK adults aged over 18 smoke – an improvement over the 20.1% that smoked in 2010 – but still representing approximately 7.6 million people in 2016. Everyone Health’s smoking cessation services are aimed at this significant proportion of the UK adult population and are designed to ensure that all service users not only quit smoking in the short term, but deliver permanent lifestyle and behaviour change, meaning they’re never tempted back.

Our highly-trained team of experts provide service users with advice, support and encouragement on how best to go about quitting smoking for good, something which studies show they’re four times more likely to do if you make use of both the specialist support and medication we offer.

The service includes one-to-one support from our experts to the service user, as well as 12 weekly group sessions to help deliver permanent behavioural change. Furthermore, each service user will also benefit from a bespoke programme that has been tailored to each of their individual needs. This includes advice on choosing the right smoking cessation medication for the service user, as well as how best to deal with cravings and withdrawal symptoms.

Service users can make appointments to see their advisor through their local GP practices, pharmacies and other community locations throughout the local area, meaning support is always within reach.

Furthermore, we also have a dedicated phone line that’s contactable on 0333 005 0095, meaning service users can talk to an adviser whenever they need to, especially if they feel their resolve weakening.

As one of the largest causes of serious injuries among the older population of the UK, helping to prevent falls is one of Everyone Health’s primary concerns. Not only do falls cause many injuries to older people, but they also act as a barrier, stopping people from maintaining their independence.

This is why we’re so keen to help by using our wealth of experience and our highly trained instructors to help combat the problem. Our falls prevention classes – which are run by the expert trainers – help service users build their lower body strength, balance and your confidence.

All of these helps reduce the chances of service users falling and, subsequently, injuring themselves, as well as helping empower older people into maintaining their independence by reducing their fear of falling. Furthermore, the programme helps older people stay active, getting them out of their homes, meeting new people and helping them improve both their mental and physical wellbeing.

Classes are held weekly and includes chair-based exercises for those who find it more difficult to stand, alongside a whole host of other exercises for service users to enjoy.

As part of the services we provide, all eligible service users will be given the opportunity to have an NHS Health Check, a simple procedure that gives excellent insight into the future health risks of service users in your area.

Who needs an NHS Health Check?

Offered to all Adults in England between the age of 40 and 74, the Health Check measures several different variables, assessing the service user’s chances of developing certain conditions such as dementia, type 2 diabetes, heart disease and some cancers.

Age is one of the biggest reasons behind the risks of service users developing these ailments goes up. As well as advancing years, however, there are several other factors that can increase a service user’s chances of suffering from any or all (developing one significantly increase the risks of getting another) of these maladies.

What does an NHS Health Check achieve?

Potential risk factors include being overweight, eating unhealthily, smoking, pursuing an inactive lifestyle, drinking too much alcohol, as well as having high blood pressure and cholesterol. By having an NHS health check, this means early signs of these conditions are more likely to be caught and the above risk factors can be mitigated and brought to heel earlier, reducing the risks of service users developing some life-threatening conditions in the future.

What does an NHS Health Check involve?

The health check itself is a simple procedure, lasting for between 20 and 30 minutes. It involves the service user answering certain questions, having their cholesterol and blood pressure checked, alongside recording their height, weight, gender and ethnicity.

What happens next?

Service users will be told precisely what their results mean and, if necessary, they will be asked to return at a later date. Service users with slightly raised blood pressure may have to have their kidneys tested via a further blood test, while another blood test may be required to check for type 2 diabetes.

Once all the checks have been carried out, service users may well be prescribed a course of treatment or medication in order to help prevent or reduce the risks of that person suffering from any of these serious conditions in later life.

Exercise has a huge range of benefits, from improving your mood to helping avoid life-limiting conditions. Exercising on a regular basis helps manage your weight, which not only improves your physical appearance, but will help reduce your risk of type 2 diabetes, heart attack, strokes and some cancers. It will also help lower your blood cholesterol and blood pressure levels. Our classes aren’t just for those who want to get fit, if you’re at risk of falls, have been in hospital or bed ridden it’s also a great way to get back into movement. We’ve made sure that some of the excises are chair based to accommodate all. As a result, you’ll develop stronger bones, muscles and joints, you’ll be stable on your feet whilst also lower your risk of developing osteoporosis. The endorphins from exercise will give you more energy, help you sleep better, make you feel happier and more relaxed.

During the session, our highly trained instructors will help you try many new activities, all aimed to be enjoyable and educational. We start each of our sessions with a warm-up, either standing, or chair based. These activities can include leg extensions, sitting to standing using resistance bands and work to mobilise different parts of the body.

If you’re feeling lonely or isolated from society, then we’re here to help. We will help those of you who are not, or don’t feel able to get out and about, seeing your friends, family and meeting new people on a regular basis.

Not only will maintaining a socially interactive life help improve your mental health and make you happier but avoiding social isolation will also improve your physical wellbeing as well. Studies indicate that loneliness can cause depression, high blood pressure, anxiety, becoming less mobile and increased mortality rates too. In fact, some studies suggest that loneliness can be as bad for you as smoking around 15 cigarettes a day.

Our highly trained team will identify if you are at risk of becoming lonely or isolated and set in motion a four-step programme to help you avoid loneliness. You’ll be allocated your own staff member who will organise home visits or regular telephone appointments with you and help make you aware of access services to help you get out and about, all the while maintaining your independence.

We can also help you make use of technology to stop yourself from becoming isolated. For example, we are running a pilot scheme using tablet computers which will enable you to use video calling technology to keep in touch with other service users, and we can keep in touch with you too. We’ll be giving out five iPads initially as part of the scheme and we will be teaching recipients how to use them to stay in touch with one another, as well as loved ones.

You’ll also be put in touch with local community groups and shown how to get in touch with the Silver Line telephone helpline should you need it.

The Healthy Schools Service provides, through its website, a single point of reference for information and updates on new programme initiatives, information on local training offers and events, useful weblinks as well as a host of resources and lesson plans to support schools embed a robust health and safety agenda.

A network of partner services, that comply with the locally agreed Quality Assurance standards, work in collaboration to provide the content which covers a wide range of topics from physical activity and healthy eating to mental health, keeping safe and relationships. The service also facilitates the delivery of multi-agency projects which are developed in response to identified needs – many of which are free of charge.

There is also the opportunity for schools to take a quick online health check which takes no longer than 15 minutes and provides the benefit of a self-assessment with sign posting to the next steps to take to help comply with the requirements of the new Health Education curriculum.

What is it?

Here at Everyone Health, helping children and young people manage their weight and supporting those who have a higher than healthy BMI is at the very forefront of the services we provide.

We offer a range of services to help support children, young people and their families through that journey, each of which is designed not just to help the service user and their family lose weight in the short term, but to help encourage whole-life behaviour changes that will result in long-term benefits to the service user.

These are flexible and can be moulded to the service user – and their family’s own needs – whatever they may be. A key element of our services is age-appropriate education. Helping the service users and their families to learn how to eat healthily and how to live a healthy lifestyle, including nutritional physical activity information. They can then apply this information to their own lives, helping them to manage their own weight and forge an independent path to success.

Kickstart:
The 12-week Southwark GP Exercise Referral service has been designed to help inactive people that are struggling with their mental and physical health. It does this by encouraging them to get more physically active both within the programme and beyond their participation and they are supported in taking more control over their own health.

There are three levels of support available: Kickstart, Active Boost and Cardiactive. Each offers different levels of support depending on what help you need.

Kickstart is the lowest level of support available and is ideal for people who are confident to exercise safely and independently. This level includes:

  • discounted membership to leisure centres for using the gym, going swimming and attending group fitness classes.
  • access to free or low cost non-gym-based activities that include led walks, cycle training, community dance and fitness courses, seated exercise and park sessions such as walking football.

Eligibility Criteria

Service users must have at least one of these:

  • Obesity BMI 30 + or South Asian 27.5
  • Anxiety +/or depression (able to self-motivate)
  • 5%  + CVD risk over next 10 years using Qrisk 2 (https://www.qrisk.org/2017/)
  • In cancer remission for up to 12 months
  • Asthma
  • Stress
  • Respiratory

What does the Programme involve?

For a 12-week period, clients will be encouraged to build up their weekly activity levels through attending sessions at the centre of their choice.

All clients will take part in an initial assessment at the beginning and a completion assessment end of their 12-week programme. Once they have completed the programme, clients will be encouraged to independently continue with regular physical activity and will be provided with information on local activity opportunities. On completion of the programme, all clients will be entitled to a reduced leisure centre membership.

Clients will be followed up at 9 months from start date on the programme and offered advice and support where needed by an administrator or exercise professional where possible.

Who is Eligible for the Service?

  • all adults aged 18 and over
  • residents of Southwark
  • or registered with a Southwark GP
  • People currently doing less than 150 minutes of exercise each week.

It’s also vital that they have no contra-indications to exercise and they must be clinically stable. Additionally, any patient with a cardiac condition can only be referred to the Cardiactive service.

Who can make an exercise referral?

  • GPs
  • Practice Nurses
  • Specialist Nurses e.g. Health Check Hub, Cardiac Nurses
  • Specialist Cardiac Physiotherapists
  • Cancer Rehabilitation Team (GSTT)
  • Renal Rehabilitation Team

Referrals can be initiated by a mental health professional by telephone or letter to the patient’s GP whose authority is required, and who can then send a referral to the programme.

Active Boost is the second level of support. This is for service users who need a little more support in boosting their confidence in managing their own exercise programme, as well as in exercising safely. This level includes:

  • get reduced cost leisure centre membership and a timetable of specialist supervised group fitness classes including gym sessions, aqua classes, circuits and Pilates.

Eligibility Criteria

Service users must display at least one of these symptoms:

  • Obesity (BMI 40 +)
  • Hypertension above 140/90 but controlled
  • Diabetic type 2 medicated or diet controlled
  • 20%  + CVD risk over next 10 years using Qrisk 2 (https://www.qrisk.org/2017/)
  • In Cancer treatment and rehabilitation

Osteopenia

  • BMD greater than 1 SD and less than 2.5 SD below young adult mean

Osteoporosis

  • BMD – 2.5 at spine, hip or forearm or >4 on fracture index with no history of previous low trauma fracture or history of falls

Osteoarthritis

  • Mild where physical activity will provide symptomatic relief
  • Osteoarthritis/Rheumatoid

Arthritis

  • Moderate OA/RA with intermittent Arthritis mobility problems

Asthma/Respiratory

  • Grade 1-2 MRC Dyspnoea scale:
  • 1 –only get breathless with strenuous exercise
  • 2 – short of breath when hurrying on the level or walking up a slight hill
  • Patients Grade 3-5 MRC to be referred into Pulmonary Rehabilitation (PR) for a 4–10-week multidisciplinary programme before referral to exercise
  • Referral Scheme (if appropriate)

Neurological Conditions

  • E.g. Young onset Parkinson’s Disease (stable), Multiple Sclerosis (Must be able to independently exercise and set up machine).

Cardiactive is the highest level of support and is for people who did the most help. This programme has been designed for people graduating from phase three cardiac rehab in the last 6 months, as well as those who have had a cardiological assessment or review in the last six months. This level includes:

  • a specialist cardiac rehab programme designed especially for each patient.
  • gym and specialist circuit sessions.
  • access to motivational support from the team
  • educational sessions and walks with other service users.

Eligibility Criteria

Patients must have at least one of these symptoms, conditions or one of these procedures:

  • Atherosclerotic disease e.g. peripheral vascular disease (PVD)
  • Coronary Artery Bypass Graft Surgery (GABG)
  • Following arrhythmias and implantable device interventions (ICD, PPM, CRT)
  • Myocardial infarction (MI)
  • Stable angina
  • Stable heart failure (HF) including chronic heart failure and cardiomyopathy
  • Undergone a percutaneous coronary intervention (PCI)
  • Valve replacement

Follow up will take place at 3, 6, 9 & 12 months.

The Healthy Schools Service provides, through its website, a single point of reference for information and updates on new programme initiatives, information on local training offers and events, useful weblinks as well as a host of resources and lesson plans to support schools embed a robust health and safety agenda.

A network of partner services, that comply with the locally agreed Quality Assurance standards, work in collaboration to provide the content which covers a wide range of topics from physical activity and healthy eating to mental health, keeping safe and relationships. The service also facilitates the delivery of multi-agency projects which are developed in response to identified needs – many of which are free of charge.

There is also the opportunity for schools to take a quick online health check which takes no longer than 15 minutes and provides the benefit of a self-assessment with sign posting to the next steps to take to help comply with the requirements of the new Health Education curriculum.
