Full Time Role (40 hours per week including paid breaks)
Everyone Health is looking for an experienced Mental Health – Health Trainer. This role will help support people on the SMI register by providing access to an annual physical health check.
Within this role you will help address the barriers which prevent service users accessing SMI physical health checks.
You will be required to reach out to any service user who have missed their appointment and to rebook them and provide support by identifying the barrier to not attending and address any concerns or issues raised.
You will provide education around physical health checks and the importance of the reviews and support service users to engage with their health checks in the home where applicable.
You will support service users to access clinics and facilitate the service user getting to the clinic for their Health Check if required.
You will be helping reduce health inequalities and exclusion by improving accessibility to checks for SMI populationSupport people on the SMI register by providing annual health checks and avoid unnecessary hospital admissions preventing, reducing, or delaying the need for long-term care.
Successful applicants will receive a DBS check, First Aid Training, and specialist service training as required. Mileage expenses will also be reimbursed.
The successful candidate(s) will:
Have a Full UK Driving and have access to a car
- Have strong time management, empathy, communication, and interpersonal skills.
- Have experience with supporting service users with SMI
- Display an awareness of key health messages around the benefits of a healthy lifestyle and managing key risk factors: smoking cessation, diet, physical activity, social isolation, weight management and alcohol consumption.
- Have a full UK Driving licence and have access to a business insurance car.
